Change of Address
When you have a change of address, you are required to provide the following documents in person to the District Registrar's Office.
- Change of Address Form provided below
- Driver's License (for identification)
- Two proofs of residency (described below)
► First Proof is a copy of the Lease/Mortgage pages showing:
Current Date
Your Name
New Address
Signatures with current date
OR Current dated Tax Billing
► Second Proof is your DMV change of address notice OR a utility bill (i.e. Electric, Gas, etc..), showing your name, current date and new address.
You will have 30 days to provide your second proof information, if not available on the "effective date". You can email a copy of your second proof to Mrs. April Burger (burgera@krsd.org), Central Registrar.
